Proof of identity
- For a woman: driver's license, photo ID card, passport or student ID.
- For an infant: birth certificate, crib card or hospital discharge paperwork.
- For a child: birth certificate, crib card or passport.
Proof of income
- Proof of all sources of income for everyone living in the household, for example pay stubs for the last 30 days, an unemployment letter or child support.
- An income tax return only if you are self-employed.
- Your ACCESS card, if you have one.
Proof of address
The state list on the WIC home page does not spell out address documents. Ask the clinic when you book; a current bill or letter with your name and Pennsylvania address is the usual request.
Already on WIC
If you are a current participant or a proxy, Pennsylvania WIC asks you to bring your WIC packet and WIC ID card to every appointment.
Questions
Do I need to bring my baby?
Bring each infant and child who is applying unless the clinic tells you otherwise; staff take height and weight at the visit.
What counts as proof of income?
Pennsylvania WIC lists pay stubs for the last 30 days, an unemployment letter, child support and, for self-employed people, an income tax return.
